General Provisions
§ 41-59-65. Application for permit to conduct membership subscription program; fees; renewals

Either a public or private ambulance service licensed and regulated by the State Board of Health desiring to offer such a membership subscription program shall make application for permit to conduct and implement such program to the State Board of Health. The application shall be made upon forms in accordance with procedures established by the board and shall contain the following:
The name and address of the owner of the ambulance service;
The name in which the applicant is doing business;
The location and description of the place or places from which the ambulance service operates;
The places or areas in which the ambulance service intends to conduct and operate a membership subscription program; and
Such other information as the board shall deem necessary.
Each application for a permit shall be accompanied by a permit fee of Five Hundred Dollars ($500.00), which shall be paid to the board. The permit shall be issued to expire the next ensuing December 31. The permit issued under this section may be renewed upon payment of a renewal fee of Five Hundred Dollars ($500.00), which shall be paid to the board. Renewal of any permit issued under this section shall require conformance with all requirements of this chapter. Any increase in the fee charged by the board under this section shall be in accordance with the provisions of Section 41-3-65.
