Sec. 2. As used in this chapter, "good faith estimate" means a reasonable estimate of the price each provider anticipates charging for an episode of care for nonemergency health care services that:
(1) is made by a provider under this chapter upon the request of the individual for whom the nonemergency health care service has been ordered; and
(2) is not binding upon the provider.
