§ 46-3-468. Application for Reinstatement of Certificate of Authority

Application for Reinstatement of Certificate of Authority To: The Secretary of State Atlanta, Georgia Pursuant to the provisions of , the undersigned foreign electric cooperative hereby applies for a certificate of reinstatement of a foreign electric cooperative and, for that purpose, submits the following: Code Section 46-3-468 First: The name of the foreign electric cooperative at the date of the certificate of revocation was . Second: The new name by which the foreign electric cooperative will hereafter be known is . Third: The certificate of authority to transact business in the State of Georgia was revoked on for failure to follow the requirements stated in .
